SQL logic error

HI All,
I start having issue everytime i do a change on freepbx not keep it when i look im getting this error

Hi Dave,
Thank you for your respond so couple days a go some extension not reeving calls but they can make calls I had them logout from the phone completely and log back in would fix the issue i start looking errors and i cam across on this I make a change on the extension like name Gui shows new name when i call from that ext show old name.
