We have extensions in homed in multiple area codes in the USA. We use the Extension Routing (commercial module) to be able to assign Outbound Routes to individual extensions. If an extension is in the (859) area code it will use this Outbound Route. The route will accept a properly dialed (for Flowroute) 1NXXNXXXXXX format, or prepend the “1” to a 10 digit number, or prepend 1859 to a 7 digit number, or remove an extra “1” from the beginning to leave the proper 11 digits.

We make no changes at the Trunk level in our setup. Note I removed our Outbound Dial Prefix to match your setup for example purposes.
