Issues interacting with an IVR on outbound Calls

I’m having an issue with interacting with a small portion of IVR systems when we make an outbound call. The system works fine for about 95% of calls that land in an IVR, but there are a few when we call no matter when we press, the DTMF does not seem to be recognized by the remote IVR system. For clarification, these IVR systems are not ours and are well established systems.

Any idea where to start checking my system to see if it is a setting issue?

Maybe one-way audio?
Have you been able to replicate the issue? Are there any patterns observed?
It could be a trunk/configuration issue. What kinds of trunks do you use?
Perhaps reviewing the logs on a successful connect vs. a problematic one?
