Installation restarts after reboot

I am trying to install the freePBX distro version :Stable-6.12.65 via. a USB img
After dependency check, formatting the drive and the package installation, the setup is supposed to reboot. It does so but does not resume at the last stage, instead restarts.

Could some one suggest a solution?

Yank the stick - after the first boot, it doesn’t need the stick anymore - DVD does the same thing.
