Currently running FreePBX 14.0.13.6
In the IVR we have set the directory to the # key and was always working. Now when someone presses the # the system hangs up. I tried changing it to a number and still the same issue. Guess it could be related to an update on the system.
How can it be fixed ?

Random guess, change your IVR to No-Legacy for Strict Dial Timeout. This was recently pushed out of edge.
So a user called me this morning saying that all my calls are going to a different user, at first I was like sure they are… well that’s persons extension was 151 and the IVR had the other person as 1, 5 so it made sense the person was probably just dialing the 151 too slow and it was going to the other phone right… NOPE I dialed in using my Cell and pressed 151 as soon as the IVR picked up. and it went to the other person… so it seems like Enable Direct Dial in the IVR isn’t working as it used…

So how many more people are going to be updating, since this was finally pushed live, and finding problems because of this poorly thought out IVR change?

Surprise, more issues with this.
Running FreePBX v14... No resent changes made, except to change the name on an extension. When some calls in, and let's say they dial 555 then end up getting 511, 511 can then forward the call to 555 no issue, but I have no idea what is causing this.
