Call rejected because extension not found in context 'from-internal'

I have set up a few Grandstream DP722s and they seem to work. I can call the extensions from the outside, and they ring, I can answer the fall, and voicemail with email also works. I have one problem though: The phone shows me a list of recent callers. I can select one and press the call button, and it is supposed to return the call. Instead I just hear 2 beeps, and the only line I see in the Asterisk log is this:

pjsip_session.c: 10: Call (UDP:192.168.0.26:45385) to extension ‘+1xxxxxxxxxx’ rejected because extension not found in context ‘from-internal’.

(The x’s are a real phone number).

Any ideas?

Confirm that you have an Outbound Route that accepts +1xxxxxxxxxx and sends it to the trunk in a format that the provider accepts.

Did not change anything. It still shows the exact same message in the Asterisk logs.
Also tried just “+.” instead of the "+1NXXNXXXXXX

I have a new problem that may or may not be related. At the moment I don’t seem to be able make any calls from my system, all end up in “all systems are busy right now”. The asterisk log shows me this error:
acl.c: Unable to lookup ‘FreePBX.mydomain.com

mydomain.com is registered and I can ping it. “Freepbx.mydomain.com” is not a computer on the network, I am using FreePbx on a different computer that is not on the mydomain network and it is using DDNS. How do I get acl to simply lookup mydomain.com instead of Freepbx.mydomain.com?

When I place a call to my MTA, they seem to go through, but get rejected, and I have the suspicion that this could be related.

Ok, I am not sure what happened, but for some reason both issues seem to have resolved themselves. Good on one hand, not good on the other as I have no idea why this happened.
