I thought I had solved this issue by deleting the astdb Database, but unfortunately at my next FreePBX 17 upgrade (apt upgrade to upgrade from asterisk 22.3.x to 22.4.x, fwconsole ma upgradeall) it broke again.
I can’t call any extensions until I manually click each pjsip extension in the admin interface and submit it - doesn’t even take “apply changes” to take effect, right after submitting the extension is reachable again.
I did not do anything special during update, the only weird thing during the fwconsole ma upgradeall was that it complained “Unable to locate the FreePBX BMO Class 'Core’A required module might be disabled or uninstalled”, but after I ran fwconsole ma upgradeall again it finished fine. Then on the dashboard it complained about the signature of a few .png files which I fixed by running the ‘fwconsole ma refreshsignatures’.

I would be deeply grateful if someone could at least point me in the right direction why my system is so broken. This is a FreePBX 17 install done in early Nov '24 that I did not restore any backups to - all stuff was manually configured by comparing it to my old FreePBX 16 system. This is the fourth time this happened to me after doing the upgrade, so it’s not a fluke
