Voicemail setup repeats each time we use it

Using FreePBX 2.11.0.0rc1.5 (All updates applied)…

Each time a user connects with voicemail (*97), the system acts as if they have never used voicemail before and begins prompting them to set up password ,etc. You set it up and next time you dial *97 you are prompted to set up again. Otherwise it is working fine. What could be causing this?

If the password is the same as the extension , that behavior can be triggered by your settings in /etc/asterisk/ m* files
