We’ve inherited a very reliable but out of date PBX.
Can somone point me to a guide on migrating to the latest OS version please?
Not sure that there is a guide that exists for your exact version but the Backup and Restore module in version 16 is meant to be able to restore a backup from very old versions of FreePBX. Just take a full backup from the old system and attempt to restore to a recent FreePBX installation.

If this is a production system and if you can wait for the upgrade I would wait for a full release of FreePBX 17 as there is no direct upgrade path from FreePBX 16 to what eventually will be version 17.
