Unable to auto provision SNOM M500 Phones

I have tried the following connection strings:

  http://FQDN.com:84/snomM400-{mac}.htm (using the 400 the template)

I have tried creating a xml but exporting from the phone. I can register the phone manually fine. Anyone know the exact connection string to use? I also don’t ever see it even hit the PBX when attempting to auto provision.
