Playback failed messages but audio ok

I keep seeing these messages on the Asterisk CLI:

[2026-10-29 12:57:23] WARNING[843820][C-00001836]: app_playback.c:523 playback_exec: Playback failed on PJSIP/Telmex_SIP-0000328d for custom/apcdmx_entrada&silence/2,noanswer

Every time someone calls into our IVR I see this message even though the audio plays fine to the user. I usually ignore it but maybe its time to find out why it keeps happening. I only see it for that particular audio file so I made sure it is properly encoded and in a supported format which it is since I can hear it when calling in. Any ideas?

That string is two files played in sequence: custom/apcdmx_entrada and silence/2. The warning is logged when any one of them fails to open, and it doesn’t say which, so the file you checked may not be the one failing. Things to try:

  • Look for the silence file: ls /var/lib/asterisk/sounds/silence/ (or under /en/silence). If silence/2 is missing for your language, Playback fails after the main audio has already played, which matches “audio fine, warning anyway”.
  • Run “core set verbose 5” and call in; the “Playing ‘xxx’ (language ‘xx’)” lines show exactly which file is attempted.
  • Confirm file ownership and permissions are readable by the asterisk user, and that a format matching the channel codec (e.g. .ulaw/.wav 8kHz) exists, to avoid transcoding failures.
