I tried various CSV format to upload Phonebook entries - no success 
Can anybody perhaps send me a sample phonebook CSV file?

Hi Tobias,
I’ve get the same trouble.
I dont no if it`s can help you but, Create 1 or 2 number first in your Asterisk Phone Book, Export it in CSV.
Then edit the CVS file in notepad, not Excel because Excel modify the format.
After that you will be able to add informsation in the file and import it.
