IVR from outside has no audio, works fine internal

I have a pfsense box which I am thinking is blocking some of the traffic. I have changed the port on the pbx to use 5061 for testing since there is already a pbx using 5060. If I use a softphone and dial 7777, the ivr works flawlessly. If I dial from the outside, I get dead air. The port is forwarded to the pbx IP on 5061. If I make the inbound route to an extension, it also works fine.

This sounds like what I ran into when I setup sipstation. That turned out to be not allowing all of the proper things through the firewall. You also need to allow ports over 10000. See the details in the wiki Using a Remote Extension
