Intermittent issue with conf participants being 'kicked out'

We had a legacy v13 system that had been fine with conferences for years start doing this a few weeks ago. We went through the backup and restore to v15 last week and we have reports that it’s still happening. I think I caught a disconnect here, but I don’t know enough to discern. It seems to happen just 2-3 times per week, so watching and waiting is difficult.

Input on logging, monitoring, fixes appreciated.

There is no disconnect here, it’s just a channel navigating an IVR.
