As I understand the 7912G is no longer sold and therefore my not now be supported. Do you know which firmware image is loaded on the phone ?

FYI: FreePBX does not have any modules to configure (aka provision) phones. Any of those that exist have been provided by distro providers as of this time.

Google is your friend. Search for setting up Cisco 7912 and asterisk. www.voip-info.org is one site that has several good examples and plenty of documentation.

Also without knowing many more details it’s not possible to provide a complete set of documents as we don’t know if you are using sccp (aka the skinny protocol) or sip, and which version of firmware, etc…

Google is really your best friend in this case.
