How can I define voicemail greeting based on if the call is originating from a local extension or external/DID line? Different greetings for specific internal and external numbers would also be of use.

Pretty sure that’s not a feature of Asterisk directly within a voice mailbox. However, you could define different call flows from inbound calls into custom announcements that then send the caller to a specific mailbox to leave a message.
Set up the ‘local’ greeting on the voicemail box. Route the external call to a Ring Group that has only one extension. Set the Destination if no answer to an Announcement with the ‘external’ greeting. Set the Destination for the announcement to Voicemail (the extension) No Message.
