Just need to confirm if this error is a hard drive failure, reading a few posts which seam slightly different suggests it is. Also had two HDD in this system which where seen but the RAID doesnt seam to have made a copy to the second one, reading up has this feature been removed in FreePBX and should i have used the MB RAID?

I believe RAID setup has to be an intentional choice; there is no default RAID setup. Those errors look pretty clearly like hard drive failure; might want to boot up in single user and run some diagnostic & repair tools.
