Ganstream configure IP phone to FreePBX?

I have a problem with the configuration of the IP phone to assign extensions of FreePX

i know the IP of my Frepbx the sip configuration, name of my extencion inded the ip adress of each ip phones.
but i can’t connect and get it work help plz.
