I installed FreePBX 2.5.1 and Asterisk 1.4
when i go to Panel, it gives a page contains:
404 Not Found
File not found.
Please how can i fix this issue or reinstall FoP separately to make the FoP working properly.

I would download the latest .30 FOP while you are at it.
From the www.asternic.org site
Download the latest version (look at the top-right corner on your screen). Untar the source: (tar zxvf filename-you-downloaded.tgz), cd into the directory and read all the files with uppercase names (README, FAQ, etc).
Copy the files in the flash subdirectory to a suitable place on your web server. If your web root is /var/www/html, you can create a subdirectory ‘panel’ (mkdir /var/www/html/panel) and copy the files there (cp flash/* /var/www/html/panel).
From Skyking - Download the tarball to /usr/src and work from that sub directory
