Extension Settings Module not working

I have not looked at the Extension Settings module in a few weeks but went to look at it today on our primary and secondary server and I can’t get the module to show any settings on any of my phones. It lists all extensions but every box is unticked (Off)

Freepbx 2.11.0.11 (1.89.210.57-2)
Asterisk 1.8

All modules uptodate except System Admin which wont upgrade above 2.11.0.23

Anyone have any ideas?

Update - Got System Admin Updated which did not fix problem.

Rolled Extension Settings back to 2.11.0.0 and it works fine. Updated to 2.11.0.1 and it does not work.

Looks like it needs to be raised as a bug - How do I go about this?
