Dialplan to route US calls to one carrier and LD elsewhere

Do I have to make Dial Rules for every single area code in the USA?
It may be 100-200 entries long… Is it sensible to have a dialplan that long?

I worry that some countries like Grand Cayman and Canada also have 9 digit phone numbers with the country code of 1 just like the USA. Maybe there could be others…

Yes, there is alot of country, JAMAICA, BARBADOS, BAHAMAS, DOMINICAN REPUBLIC. I would not worry about canada its cheaper that the usa.
