we are experiencing random dropped calls, what could possibly the reason?

Looks likw malformed/bad rtp packets possibly.

what could possibly cause these? i’ve been seeing these logs for quite a while now.

Confirm with your vendor the RTP ports they use and make sure you are using the same.
Sometimes firewalls can mess with these packets as well.

What kind of phone is extension 218? Is there transcoding going on?

checking on that, i just got hired with them so im not sure what devices they are using. but most likely this is a softphone.
