Call are cutoff while leaving voicemail message Need Help!

First I am using FreePBX ver.2.8. When I dial out of my pbx and it rings 5 or more times and goes to voicemail, but while I am leaving a message I am cutoff and the following messages is played “number-not-answering”. I am not sure why this happens and was wondering if anyone else experienced this issue and if so how did you fix it? Here is a trace from the pbx.
