I’m using FreePBX to run a small residential phone service. I have people requesting to blacklist certain numbers. The only problem is with the blacklist module it blocks them from the whole system; so maybe one person wants a number blocked but another person doesn’t. Is there a way to blacklist numbers but only for a given extension?

This is one of many great reasons why you should not use FreePBX to run a residential VoIP service.
If you are in the US you must comply with CALEA and privacy regulations, neither are addressed by FreePBX.
A2Billing is a good Open Source package that can provide some carrier and portal billing features.
What are you using for an Internet connection and how are you addressing e911?
