How can I get this feature or a replacement working so I can get the “false” outbound rings instead of silence until the other end actually starts ringing?
Appears that this problem is only on normal 10/11 digit calls that get redirected to another trixbox server via an IAX2 trunk, not on all outbound calls as I had earlier thought.

No - the problem is on calls going out a digital line (PRI, IAX, SIP, etc.) that can reach real destinations like a company IVR that is delivered to you as early media. The “r” option will block that media, resulting in ringing. In the meantime, the IVR is telling you “press 1 for new reservations, 2 to get flight status information, …” - and your users wonder why they can’t reach that Airline…
As far as the Cisco phones, I am least familiar with them over many others. However, any decent IP phone should be able to be configured to create its own ringing upon being signaled with a SIP RINGING signal (180 IIRC). The fact that you are getting silence would imply that the phone may not be properly configured. If you do a SIP debug you should be able to determine if it was sent the proper signaling to tell it to start ringing the phone.

I’ve been using ‘r’ because otherwise, when I call out from home cordless phone, there is no audible ringing. The home cordless is connected to * via an IAXy, where my SNOM SIP home office phone does hear ringing. You referred to ‘SIP RINGING’ signal, but that is presumably not available for IAX. Any ideas?
