All circuits busy on outgoing calls. No changes on pbx, route is up

https://pastebin.freepbx.org/view/83499b5e

System was fine yesterday, inbound and extension calls work, outbound results in All Circuits Busy.
So, my office can’t call out and I am headed into emergency orthopedic surgery. Any assistance would be appreciated. Stay safe, everyone.

Taking a quick look this is the first thing that I saw.

Line 154

full:[2020-03-30 11:04:40] VERBOSE[13043][C-00000022] pbx.c: Executing [s@macro-dialout-trunk:34] NoOp(“PJSIP/204-0000003e”, “Dial failed for some reason with DIALSTATUS = CHANUNAVAIL and HANGUPCAUSE = 21”) in new stack

Someone else may be able to see more in there.

See line 108, CID string includes hyphens, it prob needs to be all numeric.

Turns out my SIP provider turned off outbound route due to suspicious calls. In fact, hundreds of 12 second calls made through this PBX. We had failed to enable IP authorization and set it to the single allowed IP address.

I presume this means that someone was routing calls through my SIP provider account. They did not hack the PBX.

I have set IP Authorization to only allow this IP to route through my SIP.
I have changed all device passwords.
Anything I am missing?

Today it a security webinar so I expect to get more of an education.
