SOLVED: SQLSTATE[23000]: Integrity constraint violation: 1062 Duplicate entry '200-3' for key 'PRIMARY'

I’m using Freepbx 13.0.58 with Asterisk 11 on Centos 6.7. I’m getting the error:

SQLSTATE[23000]: Integrity constraint violation: 1062 Duplicate entry ‘200-3’ for key ‘PRIMARY’

when I save an outbound route.

If I click back on the browser, it has saved the change and allows me to apply changes.

Can anyone shed some light on the error and how I might go about fixing it?

UPDATE: I uploaded to framework 13.0.61 and I’m not getting that error anymore.

I am also having the same problem when I installed the Commercial Endpoint Manager. How was this solved?? I am dead in the water with provisioning new endpoints.
I appreciate the help.
