Ring group members, busy signal when picking up call

I’ve been beating my head on this one for a while now, so I’m reaching out in hopes that someone else has an easy answer to the problem.
What’s happening is we have an inbound DID routed to a ring group of 3 users. Randomly when one user goes to pickup the call, they will get either a busy signal or dead air, but the call continues to ring on the other two phones and when answered the caller is there. This happens about 1 to 2 times an hour.
Any suggestions are appreciated.
