retrieve_conf failed, config not applied

Ok all, Asterisk 1.8 on CentOS 5.5…FreePBX upgraded to 2.11 via module admin, however, when upgrading Parking module, got this error when trying to apply changes:
Reload failed because retrieve_conf encountered an error: 1
Added 25 seconds ago
(freepbx.RCONFFAIL)

If I run retrieve_conf I get the following problem with Parking:

I’m not a MYSQL guru, just followed the web instructions for the original installation from several web guides on installing from the source tarballs. Could some kind person please provide a little bit of help to resolve this? I appreciate your help in advance.

The culprit somehow was FOP2, stopped the service, and removed the admin module from FreePBX and my problem went away. I now have a happy system again. Sometimes addons aren’t all they’re cracked up to be.
