Remote extension

I’m trying to setup a remote extension with a pap2. I have created according to one article here a sipnat.conf with the external and internal ip. I have created my extension and it works inside the lan however when I take it out to a remote. it will not work.

Did I leave something out, anything else anyone needs. I will handle the pap2 just want to know if I missed something in freepbx.
