Problem with one extension - always anonymous

I have one extension (203) that just started identifying itself as “anonymous” on inside calls. I cannot figure out how or why this happened. The only thing that has changed has been routine FreePBX updates (I am on 2.9 RC).

Running Asterisk 1.8.2.3.

I deleted and reinstalled extension 203 in FreePBX.
I wiped and reconfigured the phone.

Still same problem.

I did a factory reset on the phone, flashed new firmware, reset the entire configuration again.

Now it works. No idea why it broke.
