Outbound callerid shows up as EXTEN and not the CallerID set in Freepbx

Ill be looking through the call logs on REPORTS and every so often its showing EXTEN and not the CallerID.

This doesnt happen every time. and I see no pattern to it. but when I have people call out sometimes thier outbound callerid will be the extension number they are using and not the CallerID setup in Freepbx.

Not only is it anoying to my users, but could be a MAJOR issue if the same thing happens during a 911 call and it didnt use the right CALLERID.

Has anyone else seen this?
