Offline Extension - Peer / Contact Blank

I created a new extension, and it is showing offline after a few minutes. I can make outbound calls to other extensions, however, no in bound calls. I looked in the Asterisk Reports down lower in the Peer section under PJSIP all extensions show a contact line, except for the one that is offline / unavailable. Where would I enter this information? It has been over a year since I set the system up and I can not recall ever inputting this information for the others.

In the phone. The phone has not (successfully registered).
