Odd call transfer problem

Here is the problem, I make a call to an outside line, talk with the receptionist, and the receptionist transfer me to the person I’m wanting to talk with. At this point my phone system transfers me to a local internal extension 100 on my system. I have have had this same problem 3 times. How would I go about debugging this problem?

I have a PRI line connected to a cisco2821, and the phone I used was a Yealink T42G.
