No inbound CID name

Ever since I upgraded to 2.2.1, the callerid name reports as the callerid number except when routed through a queue.

For instance, if John Doe at 213-555-1212 calls in on a direct DID extension it will come across as:
(I have verified that this happens if using Inbound Routes instead of direct DID as well)

But if John Doe calls and gets routed into a queue the caller id will come across as:
John Doe

Does anyone know how to fix this?

better yet does anyone else have this problem? Not sure that it matters but I am using a PRI for the inbound calls
