I was just about to tell you the same thing. I disabled direct media and it worked. Thanks so much for your help.
Direct media is controlled in the GUI when editing a PJSIP extension, Advanced Tab:
[root@lorne14-pro tftpboot]# asterisk -x "pjsip show endpoint 5004" | grep direct_media
direct_media : false

My gosh, you’re right. When did that get put in?
IMO, it should probably default to No. In the cases where it obviously won’t cause trouble e.g. on-site PBX with local extensions, it really doesn’t help much. Where it’s important, most users aren’t set up for it to work correctly.
Of course, pjsip should be smarter. A simple fix for now, if it even ‘smells’ NAT, it should not attempt direct media.
