How do IPs get in IPTables Chain fpbxblacklist

In FreePBX 14.0.16.11, fully updated, I have IPs that are whitelisted in firewall and fail2ban but occasionally wind up in fpbxblacklist. They are not listed as blocked in the firewall GUI or fail2ban. I have to find them with iptables -L | grep 123.123.123.10 then delete them.

How do IPs get in the chain fpbxblacklist ? Anyone know how to prevent this from happening?
