MySQL is stuck at 13.1%
SWAP is 38%

I believe the above 2 parameters are worrisome. are they? What action should I do?

Not really, swap and memory is a linux thing, (well handled) but press 1 while ‘topping’ you have here a load average of 2.24 in the last minute, a little less in the last 5mins and 15mins. If you have 4 cores you are likey good, if just one, you are ‘overloaded’

Server has 4GB RAM & 2 Cores.

I guess not required to bump up the server specs at this time?
