HELP! Fail2ban Broken

Somehow I have broken fail2ban, something has gone astray.
This morning I got an email that told me that fail2ban had banned my servers public IP address for SIP. I don’t know how anything would try to register from my public IP address to my public IP address as the router does allow it.
Soon after all of the SIP endpoints would get fast busy/ service failed message.

When I logged into the system admin to try and whitelist my own IP in the intrusion detection tab the page is broken. I get the attached screenshot.
Any idea what went horribly wrong? I have already rebooted to no difference in fail2ban but the phones work now.
The only thing that has changed in a moth was last night I installed “yum install minicom” and it had 2 dependencies.

This is an up to date FreePBX 12 Distro
PBX Firmware: 6.12.65-30
Asterisk (Ver. 11.19.0)

You need to update:

The fix should be:

su - asterisk -c “gpg --refresh-keys --keyserver

That ticket sorta went off on a different tangent at the end.
