Echo on internal SIP to SIP extensions - Sangoma phones

PBX Firmware:12.7.6-1904-1.sng7
PBX Service Pack:1.0.0.0
Sangoma 400, 405 and 505 phones
Vega 200G gateway for PRI trunk

Users are reporting echo especially at the start of an internal extension to extension calls between Sangoma phones. Echo will go away and return randomly during a call.
Echo sometimes happens on external calls - very random.
How do I resolve this.

Are the phone’s on the latest firmware?

The phones are on 1.54 - latest shown in Endpoint Manager
