Distorted calls Inbound, No voice Outbound v16.0.33

Hello all,
I recently upgraded from chan_sip to pjsip and calls are not working properly. I am getting told by the receiver I sound robotic/distorted when dialing in, but I hear the receiver just fine. On outbound calls there is no audio at all both ways. I have done a clean install of v16 with asterisk 19. Updated codecs to ulaw/alaw/G722. Checked the router and ports are opened for RTP and UDP. It’s worked in the past, I’m just not sure what I’m missing. I recently updated the phones setting for RTP transport to yes (grandstream) and now get the following. Any help on where to start is much appreciated.

ISP router > Cisco Router > Switch (phones connected) > Win Server > VM freepbx

Before the change I analyzed the logs and there was a bridge and no errors. but same audio issues.

UPDATE
I went through and saved my extensions in freepbx (changed nothing) and applied changes. This now allows calls to go through. Inbound calls are set to ringall using a Ring Group.

outbound log captured, but still no sounds both ways.
