Not really that quickly. You have to push things somewhat far for it to really be a problem, and at that point FreePBX itself may encounter other scaling issues so in practice it’s not an issue for the vast majority of people. If it does become a problem you either have to solve that while keeping media going through Asterisk, or if someone really wants to then they have to tightly control the environment to have direct media reliably work.
You’re conflating two different things. A telecom network is not like your office PBX network. Not all PBX systems sit in the same building with the users. Some exist only in the cloud for a business. Some have a mix of local and remove devices, WebRTC, softphones, etc.
If you want direct media where the RTP is flowing directly between two devices, you will need to configure the PBX to not use anything that would cause direct media not to work. So not DTMF based features, no call recording, no transcodings, etc. So basically you’d need to turn off standard functions in a PBX.
It’s quite likely that, if you turn off anything that frustrated direct media, you would be better off using a SIP proxy, rather than a PABX, although there are some PABX type functions that don’t require access to DTMF or speech paths, or only need them for a small proportion of the time.
