Blind Transfer Call Recordings Full of "Noise"

Only when blind transferring a call, our call recordings are unintelligible because of “noise”. Non-blind-transferred calls record perfectly.

We are running FreePBX Distro 2.11.0.10 with Asterisk 11 on CentOS 6. Twelve CPU cores, 128Gb memory

I’m not sure what other information to provide? Any ideas? Thanks!
