Authentication errors

FreePBX Distro version 5.211.65-8
FreePBX 2.11
Linux 6.5
Asterisk 11
Fully updated as of 4.4.14

Firstly, my apologies if this has already been answered anywhere else, I’ve performed a search and discovered similar posts but nothing relating to this.

We’ve built many PBX’s using FreePBX Distro and this is the first time we’ve had this error:

Connected to Asterisk 11.8.1 currently running on testpbx (pid = 2906)
[2014-04-07 11:24:47] NOTICE[25926]: manager.c:2623 authenticate: 127.0.0.1 failed to authenticate as ‘admin’
[2014-04-07 11:24:52] NOTICE[25927]: manager.c:2623 authenticate: 127.0.0.1 failed to authenticate as ‘admin’
[2014-04-07 11:24:57] NOTICE[25928]: manager.c:2623 authenticate: 127.0.0.1 failed to authenticate as ‘admin’
[2014-04-07 11:25:02] NOTICE[25940]: manager.c:2623 authenticate: 127.0.0.1 failed to authenticate as ‘admin’
[2014-04-07 11:25:07] NOTICE[25941]: manager.c:2623 authenticate: 127.0.0.1 failed to authenticate as ‘admin’

I’ve checked manager.conf to confirm the password is correct, and it matches the password shown under Advanced Settings so I’m puzzled as to what could be attempting to authenticate and is failing, any ideas?

It’s a local process based on the loopback IP.

It was indeed FOP2, although strangely I’ve never had to manually amend the fop2.cfg file before but this time it was still showing the default amp111 password. I changed it to match the correct password and the auth errors ceased.
