We only see the problem when initiating a few hundred calls with call
files and having peoples doing manual dial at the same time.
I am investigating a patch in res_agi in asterisk more to come.

RH9. I am running the same setups (AMP 1.0.010 and * 1.0.10) and never
found any problems.
The system is Debian sarge with Linux 2.6.12.
